Accuracy of information: I confirm that the information I have provided is complete and accurate to the best of my knowledge. I will notify Adventis Travel promptly of any changes or errors.

Authority to book: I confirm that I am at least 18 years old and authorised to make this booking and provide the necessary information for all travellers included in it. If the lead traveller is under 18, a parent or legal guardian must provide their details and sign this form on the minor's behalf.

Booking authorisation: I authorise Adventis Travel to submit the reservation requests and coordinate the travel arrangements agreed with me in writing. I understand that submitting this form does not itself guarantee a reservation.

Supplier terms: I acknowledge that accommodation, transport, activities and other travel services are provided by independent suppliers, each of which may have its own terms, restrictions and conditions.

Price and payment: I acknowledge the agreed quotation, pricing, payment schedule and applicable fees set out in my invoice. I understand that availability and pricing may change until the relevant supplier confirms the reservation under its applicable terms.

Cancellations and amendments: I acknowledge that I have been given access to the applicable cancellation, amendment, refund and no-show conditions before authorising the booking, and understand that charges or losses may apply.

Travel documentation: I understand that I am responsible for ensuring that all travellers hold valid passports, visas, inoculations, permits and other documents required for their travel, and for checking the applicable entry requirements.

Travel insurance: I understand the importance of appropriate travel insurance and agree to ensure that all travellers have suitable cover for their trip, including medical emergencies, evacuation, cancellation and interruption where available.

Privacy: I acknowledge that Adventis Travel will process the personal information provided in accordance with its Privacy Policy for the purposes of arranging, administering and supporting my booking.
